Under Support at Home, care is planned with you and centred on your needs, goals, culture, and preferences. Your Help Plan records what you want to achieve, the services you will receive, who provides them, how often they occur, any risks and how they will be managed, and when the plan will be reviewed. It is developed collaboratively with you and any supporters you nominate, and it is updated whenever your needs change. Your care partner provides ongoing care management, including regular check-ins each month, so services remain safe, high quality, culturally safe, trauma aware, and delivered with dignity and respect. If you choose to self manage, you can take on more day to day decisions such as choosing workers and scheduling, while your provider maintains oversight to make sure your choices are supported safely.
