Agree an exit date with your current provider. Let them know you’re changing providers and confirm the cessation date together.
Enter a new service agreement with Five Good Friends. Once you sign, FGF (as the incoming provider) submits an entry notification to Services Australia so your services can start.
Your approval and budget move with you. Your Support at Home approval and budget transfer to your new provider. If you previously had Home Care Package unspent funds, they also follow you: the Commonwealth portion is held by Services Australia.
Finalising with your old provider. They continue services up to the day before exit, then finalise claims and issue your final monthly statement/invoice.
