Your Support at Home statement helps you understand:
• What funding you’ve been approved for
• What services you’ve received
• How much funding has been used
• How much funding remains available
This statement is not a bill. You do not need to pay anything when you receive it. If you have a participant contribution to pay, you will receive a separate invoice.
Financial summary
The financial summary gives you a high-level overview of your Support at Home funding.
Statement period
The month the statement covers. Statements are issued monthly.
Budget name
The types of Support at Home funding you’ve been approved for.
Spending and remaining balance
For each funding type, you’ll see:
• What was spent in the previous month
• What was spent this month
• How much funding is still available.
Applicable date range
This shows how long that specific budget can be used for. Some
budgets are time-limited, while others run quarterly or until the
funds are used.
Ongoing quarterly home support summary
This section covers your day-to-day support services under the everyday,
independent and clinical categories, such as:
Personal care
♥ Domestic assistance
♥ Nursing
♥ Domestic assistance
♥ Allied health, etc...
The date range for this statement. The statement period is monthly.
The date range shown here is the budget period.
This section shows the funds from entitlements and any applicable spending from previous months, which make up the opening balance for this statement.
Each service delivered during the month appears as a line item.
The remaining balance shows what’s left for the rest of the quarter.
Short-term pathways
These are specialised, time-limited supports that sit alongside your regular services:
Restorative Care, Assistive Technology & Home Modifications or End of Life pathway.
What’s different about these budgets
Each pathway has its own budget period
They appear separately from your ongoing home support
Funding must be used within the approved time frame.
Good to know:
Not everyone will have these pathways. They only appear if the funding
has been approved by My Aged Care. If you need to access short term
pathway funds when the need arises, get in contact with your care team
at Five Good Friends.
Short-term pathway statements
Assistive Technologies & Home Modifications, Restorative Care and End of Life pathway statements vary in funding and time, but follow a similar format as below.
The date range for this statement. The statement period is monthly.
The date range shown here is the budget period – this can differ from other budgets.
This section shows the funds from entitlements and any applicable spending
from previous months, which make up the opening balance for this statement.
Opening balance from your approved pathway budget.
Prescription, is the cost for the time a health professional spends understanding
your needs and recommending the right assistive technology for you.
Members may need extra services or training to ensure their equipment is safe and suitable for use. These are called wrap-around services.
Any adjustments from previous months.
The remaining balance for the rest of the pathway period.
Provider and government-held unspent funds
If you previously had a Home Care Package, you may still have unspent funds.
Important things to know
These funds do not have an expiry date
Provider-held funds are used first
Government-held funds are only used once provider-held funds are exhausted.
Opening balance of unspent funds.
Any services delivered that used these funds.
Remaining balance (if any).
Common Question: “Why did my balance suddenly change?”
This usually means provider-held funds have been fully used and
spending has moved to government-held funds.
Budget expense pages
After each budget summary, you’ll see a detailed expense breakdown for that budget.
The date (and time if applicable) the service was delivered.
Who delivered the service.
Total cost of services.
The total subsidy the government pays.
Your contribution amount (if applicable).

statement. If you need to pay anything, you’ll receive a separate invoice.
Care management
Care management helps plan, coordinate, and review your care.
Hours are shown for your information only.
Costs come from a separate care management pool held by Services Australia, you do not pay anything for this.
Restorative care management – this will be the same structure for the End-of-Life
pathway care management.
The time spent planning and coordinating your care. Unlike the above ongoing home support care management, both the hours and associated costs are drawn down from the Restorative Care or End-of-Life budget (depending on your funding), so you can see exactly how your funding is used.
